Alliance Care Management


How does Alliance Clinical Team (ACT) help providers and patients?

  1. Supports the provider's medical treatment plan
    • Creation of a patient centered Care Plan aligning patient goals with provider treatment plan
    • Multi-disciplinary approach, utilizing ongoing bi-directional communication between teams
    • Each participation provider location has access to RN or SW care managers and other SAHA team members
       
  2. Increases support and coordination for patients
    • Collaboration with other Population Health initiatives and programs to reduce duplication of efforts and facilitate consistent, streamlined patient communication
    • Coaching and education to facilitate maximum patient self-management
    • Targeted patient outreach: ongoing assessment of goals and barriers, development of patient-specific interventions
    • Focuses on high risk patients or patients experiencing challenges/needs with new conditions or recent medical events
       
  3. Promotes where to go for care with the right timing and quality
    • Facilitates communication and medical record transparency among specialists and locations
    • Focuses on patient access to the right level of care, at the right time, in the right location

About the Alliance Clinical Team

The Alliance Clinical Team relies on patient support from RN Care Managers and Social Work Care Managers trained in high risk care management support. Members of the clinical team are patient advocates and act as partners and extensions of the clinic, provider, and all members of the multidisciplinary team. Care management plays an advanced clinical role and is recognized as an essential component of population health initiatives for both Trinity Health and Saint Alphonsus Health System. Care Managers and providers work together to meet the population health goals of enhanced care coordination, improved patient outcomes and decreased healthcare costs.
