What Do I Need To Bring To My Appointment?

We strongly recommend that you bring the following information to your first appointment.

All current medications. It is preferable that you bring all bottles and containers.
Copies of medical records, x-rays and any reports from your previous physicians.
Insurance information
Personal identification information such as, Driver’s License, or ID with current picture
Copy of Advance Directive or Living Will
