The Performance Improvement Team works alongside payor partners and affiliated practices, in addition to the Saint Alphonsus Medical Group, to lower total cost of care, improve quality scores, reduce overall inpatient utilization, and improve documentation and coding of all clinically relevant active and chronic conditions that impact the care of patients.

Provider Experience
In addition to education and resources to assist providers in improving care and promoting appropriate selection and utilization of resources to optimize outcomes, the Alliance can provide the following:
- Data collection through efficient technology, analysis, and validation
- Benchmark and Annual Reports to compare to cohorts in the Alliance
- Best practices and Evidenced-based Medicine protocols that align with credible sources
Patient Experience
When providers are practicing standards of care and following evidence-based and best practice guidelines, the impact to patient care, care transitions (e.g., from one healthcare setting to another), and health outcomes is positive. Areas for direct impact may include:
- Patients will have improved access to healthcare services
- Patient education and/or counseling
- Chart reviews
- Reducing and/or preventing risks and further health complications
- Disease state management
Data Expectations
Sharing data with the Alliance drives changes. With pay-for-performance and other quality and outcomes-based reimbursement incentives, the Alliance will collaborate with providers on how to collect and validate data for accurate reporting, so this process becomes a routine effort.
As a participant in the Alliance, the expectations for your priority measures include:
- Share EMR access to the Alliance and/or
- Provide patient data to the Alliance
- Participate in quality improvement training/educational activities as necessary
The Alliance is required by federal law to adhere to HIPAA and other applicable laws and regulations. Any business and provider and patient level information will be highly secured.
